A Skinwalker is what an Anomaly becomes when you let it inside. If you admit a disguised patient by mistake, it will eventually drop the disguise, awaken into a hostile creature, and hunt you and your patients. Skinwalkers are the most dangerous enemy in the game and the only one you fight with weapons.
How Skinwalkers awaken
Once admitted, an Anomaly awakens at a random point — sometimes right after check-in, sometimes only after it has wandered into the treatment rooms. There is no countdown you can watch. When it awakens, it actively seeks out you or other patients to attack, and every attack costs a large amount of Sanity and can kill patients (which also hurts your score).
This randomness is exactly why a missed Anomaly is so dangerous: you cannot relax just because nothing happened immediately. The threat is live until you deal with it.
How to beat a Skinwalker
You have several options, from best to last-resort:
Weapons (best)
Approach with a gun or taser and use M1 attacks. This is the safest and fastest method, and the reason you should always keep a weapon in your kit by the mid-game. A starting taser sits next to the cameras at your desk, and you can buy more guns and tasers in the shop.
The wrong-item kill (cleanest)
If the Skinwalker (or the Anomaly before it fully awakens) is in a treatment room, give it the wrong treatment item. Because Anomalies dying does not cost you points, this kills it with zero scoring penalty — often the cleanest solution of all. See the Treatment guide for details.
Bare hands (last resort)
If you have no weapon:
- From behind: hold E behind a Skinwalker that is attacking a patient to take it down.
- On you: if it is attacking you, tap E repeatedly to fight it off — but this deals Sanity damage, so only do it when you have no better option.
Prevention is the real strategy
Every Skinwalker in your hospital started as an Anomaly you admitted. The most reliable way to “beat” Skinwalkers is to create fewer of them by running tight detection at the desk. A clean check-in routine means fewer awakenings, less Sanity lost, and fewer patients killed. Combat should be your backup plan, not your main plan.
The unavoidable Skinwalkers
Some Skinwalkers cannot be prevented. The Ambulance event after shift 4 auto-admits Anomalies that will awaken regardless of your detection. The Barney event can also turn into an Anomaly. For these guaranteed threats, preparation is everything: own a weapon before shift 4 and keep coffee in reserve so you can absorb any Sanity hits while you clean up.
Stay calm under attack
When a Skinwalker awakens, the worst thing you can do is panic. Locate it, choose your method — weapon if you have one, wrong-item if it is in a room, bare hands only as a last resort — and deal with it deliberately. Frantic bare-handed flailing bleeds Sanity fast. With a weapon ready and a calm head, even an awakened Skinwalker is a manageable problem rather than a run-ender.
