In Animal Hospital, you do not have a traditional health bar — you have Sanity, and protecting it is the whole survival game. Every threat in the hospital ultimately attacks your Sanity, and when it reaches zero your run is over. This guide covers every way Sanity drains, every way to restore it, and how to protect both yourself and the patients you are responsible for.
How Sanity drains
Understanding the drains lets you avoid most of them on purpose:
- Admitting an Anomaly is the single biggest drain. Once a missed Anomaly awakens into a Skinwalker and attacks you or a patient, you lose a large chunk of Sanity. Prevention at the desk is far cheaper than dealing with the consequences.
- The camera dark-figure zoom. When the camera zooms toward a dark figure near your booth, watching it drains Sanity and can be fatal. Look away immediately.
- Cursed photos. Looking at a cursed photo (bloodshot eyes and a grin) costs Sanity, and the loss is unavoidable once you look — which is why you only photograph patients who passed the earlier checks.
- Environmental enemies. The Bed Monster, Stalker, Don’t Look Up ceiling threat and others all deal Sanity damage if mishandled.
- Events. The Death Ritual costs 1 Sanity per candle, and room fires deal 1 Sanity per second you stand in them.
How to restore Sanity
The primary tools are coffee and food from the shop. Buy them between or during shifts and consume them to top up. The key habit is to keep a buffer: do not spend down to your last coffee, because some Sanity losses (like cursed photos or a candle-heavy Death Ritual) are unavoidable, and you want recovery ready when they hit. Later you can unlock a second coffee machine through the Barney event, which makes Sanity far easier to manage.
Protect yourself
Most self-protection is simply good detection. The cheapest way to keep your Sanity high is to reject Anomalies at the desk so they never become Skinwalkers inside. Beyond that:
- Break eye contact with the camera the instant it zooms toward a dark figure.
- Skip the photo entirely when you have already seen a visual or camera tell.
- Carry a taser or gun so that when a Skinwalker does awaken, you can put it down quickly instead of taking repeated hits.
Protect your patients
You are not only keeping yourself alive — your score depends on patients surviving too, and a patient killed by a Skinwalker is both a scoring loss and a Sanity hit. To protect them:
- Reject Anomalies before they get inside in the first place.
- Respond fast to emergencies: carry fainting patients to a bed within the time limit, extinguish patients on fire and then treat their burns, and stop the Death Ritual before the candles finish.
- During Surgery, ignore the tentacles and just finish — attacking them kills your patient automatically.
Build a survival rhythm
Survival is less about heroics and more about rhythm. Run the detection routine on every patient so almost nothing gets inside. Keep coffee in reserve. Handle emergencies the moment they appear instead of letting timers run down. And never panic — most Sanity disasters come from rushing, staring too long at a hazard, or letting a small problem snowball. Slow, deliberate play keeps your Sanity high and your shifts long.
