Surgery Room Treatment Guide

How to complete Surgery in Animal Hospital: the timed on-the-spot mini-game, why you can't leave, and how to deal with Surgery tentacles.

Surgery happens in room 8, on the left side of the hospital, and it is the most demanding treatment in the game. Unlike every other room, Surgery is timed and cannot be left once it begins. Get this one right and you have mastered every treatment type.

What makes Surgery different

In Basic Medical, X-Ray and Heart Monitor, you can step away to multitask and the patient waits. Surgery does not work that way:

  • It must be completed on the spot within a time limit.
  • Once you begin, you cannot leave the room until the operation is finished or the patient dies.
  • The good news: the timer does not start until you interact with the patient to begin, so you can walk in, get oriented, and only commit when you are ready.

Because of this, treat Surgery as a focused task. Do not start it when an emergency is about to demand your attention elsewhere — finish or defer your other obligations first.

The Surgery mini-game

Surgery is a follow-the-screen mini-game where you take and administer items from the room. Unlike the other rooms, Surgery has a few extra tools — such as scissors and scalpels — located in the room with you, and you match them to the pictures shown on the screen. Just follow the prompts in order.

When you reach the step where you must administer three items at once, the order does not matter — but, as in every treatment, administering a wrong item makes the patient die and costs you points. So even under the time pressure, accuracy on the item choices is what counts.

Surgery tentacles

The signature hazard of room 8 is the Surgery tentacles. These spawn during the operation and can be alarming, but the correct response is counter-intuitive:

  • Do not shoot them. Do not taze them. Attacking the tentacles with a weapon causes your patient to die automatically.
  • Just finish the Surgery while avoiding the tentacles. Complete the mini-game and they cease to be a problem.

This is the opposite of how you handle a Skinwalker, so it is worth burning into memory: tentacles are ignored and out-lasted, not fought. More on this in the wall and ceiling threats guide.

After Surgery

Once the operation is complete, Surgery behaves like the other rooms: you can leave and let the patient recover while you handle something else — unless an event is active. The hard part is over the moment the mini-game is finished correctly.

The wrong-item trick

If a missed Anomaly somehow ends up in Surgery, the wrong-item logic still holds — Anomaly deaths cost no points. But Surgery is high-pressure and you cannot leave, so it is usually better to catch Anomalies at check-in and never bring one into room 8 in the first place.

Mastering the toughest room

Surgery rewards preparation and calm. Enter only when you can give it your full attention, follow the on-screen steps, get the item choices right, and out-last the tentacles without attacking them. Do that and you will clear Surgery consistently. With all four treatment types under control, your shifts come down to two things: clean Anomaly detection at the desk and steady Sanity management in between.
