Death Ritual Event Guide

The Death Ritual is a timed emergency that can erupt in any of your treatment rooms. It is one of the more Sanity-punishing events because the cost scales with the number of candles — making preparation the difference between a minor scare and a dangerous drain.

What triggers it

Any patient — Anomaly or not — can randomly start a death ritual in a medical room. There is no warning beyond the emergency appearing on the right of your screen. When it does, you need to respond immediately, because it is on a timer.

How to stop it

The Death Ritual works like this:

1. Go into the room where the ritual is happening.
2. Extinguish or interact with all the candles within 60 seconds.
3. You lose 1 Sanity per candle as you do.
4. Once completed, the patient continues their recovery normally.

So the task itself is simple — put out every candle — but the Sanity cost is the real threat, especially if there are many candles or your Sanity is already low.

Keep coffee ready

Because each candle costs 1 Sanity, a candle-heavy Death Ritual can drain a meaningful chunk of your bar in one event. The standard advice is to have a coffee ready before you start clearing candles, so you can immediately top up if the ritual takes you dangerously low. Prioritize it as a timed emergency

The Death Ritual has a 60-second timer, which puts it among the urgent emergencies alongside fainting patients (also 60s). If it appears while you are mid-treatment or dealing with an untimed room fire, the Death Ritual generally takes priority because it will fail if ignored. Learn to recognize it instantly on the emergency feed so you do not waste any of your 60 seconds.

Move efficiently

Since the ritual happens in a specific room, knowing the hospital layout lets you take the shortest path and even sprint there, preserving precious seconds for the candle-clearing itself. The faster you reach the room, the more time you have to extinguish candles before the timer runs out — important when there are many of them.

Stay calm and clear methodically

The Death Ritual looks chaotic — a room full of candles and a ticking clock — but the response is methodical: get in, clear every candle, watch your Sanity, and sip coffee if needed. Do not panic-skip candles or leave early, because the ritual must be fully completed for the patient to resume recovery.
